Proxy Form | The SoR Extraordinary General Meeting, 19 November 2026

The Society of Radiographers' Extraordinary General Meeting (EGM) will take place online on 19 November 2026.

This form will allow our members to indicate their voting intentions for anyone who wishes to vote via proxy.

To be valid and effective, a completed and signed proxy form must be submitted before 6pm (18:00h GMT) on Tuesday 17 November 2026.

The Society of Radiographers will use the details supplied by you in this form for the purpose of fulfilling your voting intentions via proxy. Our lawful basis for processing your information is to manage your submission and fulfil our legitimate interest as a professional body. We will retain all information you submit as verification of your participation.

Your responses will be collected and stored by Alchemer who are based in the United States; data is hosted in the EU, but only made available to the account holder (The SCoR). All information collected is kept confidential and secure and is not shared with any third parties by Alchemer. Your personal information will not be viewable by reviewers.
