Thank you for signing up to the Young Persons Welfare Ambassador Programme!
This form should be completed by the Supporting Adult who will be responsible for the YPWA programme in their venue.
You will need to complete this form in one sitting - the information you will need to have available is:
- your name and contact details
- details of your venue/organisation,
- name and contact details of the young person(s) taking on the YPWA role
- clothing sizes for the Welfare Officer and young person(s)
- card details for the YPWA programme fee (£20/per young person)
The Supporting Adult will need to be on the committee/management or be in a position to engage and influence the committee/management. Most Supporting Adults are usually the Welfare Officer or Safeguarding Lead, but can be someone else depending on what works best for your organisation. The Supporting Adult will also:
- Support and engage the YPWA during the programme
- Ensure the LTA Safeguarding survey is disseminated within your organisation at the beginning and end of the programme
- Complete some short online training
- Receive ongoing support from the LTA Safeguarding team
- Network with other YPWA venues
- Receive the YPWA welcome pack
It should take you around 5 - 10 minutes to complete this form, depending on how many young people's details you need to provide.
