Request for Minor Adaptations
This form is intended for
healthcare professionals and colleagues to request minor adaptations for customers with a disability or long-term health condition. You will be asked a series of questions, some of which will have mandatory fields to ensure your request can be processed.
We can supply and fit minor adaptations such as grab rails, lever taps, key safes, and handrails at no cost to our customers.
Please note that for external handrails, we can only provide up to 4m in length. If more than 4m is required, please refer the customer to their local authority for grant funding.
For
weight-bearing equipment, it is essential to provide the customer’s weight; otherwise, we cannot proceed with the request. Additionally,
photographs are very useful and can help speed up the process.
Please ensure you have the customer's permission before submitting a request on their behalf. The information provided will be processed securely and may be shared with a specialist contractor if the adaptation cannot be completed by our own staff.

Important: This form must
not be used for mobility scooter adaptations, as these are not adaptations that LiveWest install.
Wherever possible, please complete and submit the form on the
same day you start it.
