Please note, this form is intended to be used by healthcare professionals, to request adaptations for LiveWest customers with a disability or long-term health condition. We can supply and fit minor adaptations for our customers such as grab rails, lever taps, key safes and handrails. Our customers are not expected to pay for this service.
The information you provide will be processed in connection with your request for aids and/or adaptations and will be kept securely on our systems. LiveWest may share a limited amount of this information with a specialist contractor involved in fitting these adaptations if it cannot be completed by one of LiveWest’s own staff.
