Lung Health @home – Horizon scanning

Lung Health @home – inviting systems to share innovative practice and promising approaches 

Lung Health @home (a newly established area of work) is seeking examples of innovation and new approaches being used to support people with a lung condition.

Part of NHS @home, Lung Health @home aims to support more people to manage their lung condition at home and increase access to pulmonary rehabilitation (PR) for people with moderate to severe breathlessness. This work is being led by the Clinical Policy Unit and NHS @home with a range of partners, including the Academic Health Science Networks (AHSNs). As part of this approach, we are collaborating with a wide range of stakeholders, including the respiratory networks and Transformation Directorate to align priorities and build on existing workstreams. 

We are particularly keen to hear about approaches that have increased access and uptake of PR and increased access and participation in supported self-management activity, education, exercise, or other supported activity for people with a lung condition. This could include (but is not an exhaustive list):

  • Use of approaches and/or technology to support referral and assessments for PR, such as asking patients to complete questionnaires ahead of the assessment process to reduce time in the appointment with the clinician
  • Examples of optimising the workforce in the pathway to reduce pressure and increase capacity for clinical staff, such as utilising Exercise Therapists, Advanced Care Practitioners, Care Coordinators, Social Prescribing Link Workers, and Health and Wellbeing Coaches
  • Examples of a system-wide approach, such as those using community diagnostic hubs and referrals from targeted lung health checks (for diagnosis and PR)  
  • Interventions and/or digital tools that increase access and participation in supported self-management, education, exercise, and other activities, such as text messaging with information about appointments, self-management, preparing for appointments, and remote monitoring
  • Prioritisation of patients on the waiting list for PR
  • Interventions that reduce health inequalities and increase access to PR and/or community-based self-management support  

Healthcare professionals and respiratory teams can share examples of innovative practice by completing the below form. The Lung Health @home delivery partners will follow-up on areas of interest to understand more about/learn from their approach. Findings will inform the next steps of the Lung Health @home approach and support the sharing and development of tools and resources. 

Please note that in this phase of the work (phase 2) we are looking for innovations that include rigorous evidence, so they can be included in a 'Community of Practice'. We will not be providing the level of follow up or testing with innovators as was included in Phase 1, please be as detailed as you can in your description of your innovation / approach. 

Findings will inform the next steps of the Lung Health @home approach and support the sharing and development of tools and resources.
