Respiratory Protection Equipment - Fit Testing and Training

Following the roll out of the JSP half face respiratory protection equipment (RPE), Unison Health and Safety is carrying out a survey to establish the consistency of the testing and training. We would appreciate you taking time to complete the survey which is 100% anonymous and should take less than 2 minutes to complete. Thank You.

2. When was your fit testing & training carried out?
3. How long did the fit testing and training take?
 
4. Were you instructed that when the mask is initially removed from its packaging, that the diaphragm must be removed, wiped with an approved wipe that must leave the diaphragm surface wet for at least 2 minutes and any residual disinfectant removed with fresh water before being replaced?
 
5. Were you issued with the mask you wore for fit testing?
6. How many attempts did it take in order to pass the Fit Test?
7. Were you informed by the trainer when and why you should wear the JSP mask?
8. Were you tested whilst breathing normally?
9. Were you tested whilst breathing deep?
10. Were you tested whilst reading a section of text?
11. Were you tested whilst standing and bending forwards at the waist?
12. Were you tested whilst moving your head side to side?
13. Were you tested whilst moving your head up and down in a nodding motion?
14. Were you informed to test your mask at the start of every shift?
15. Were you informed that the filter should be replaced when you feel resistance to your breathing or after 28 days following first use of the filter?
16. Does your JSP mask have a tag with your payroll number on it, identifying it as yours?
17. Have you been issued with your personal Log Book to record all filter first use dates?
18. Did either of the filters break when being fitted to your mask?
