UNISON Bexley branch disabled members' survey

My name is Victoria Baker. I work in LB Bexley Children's Social Care as a social worker and I am UNISON Bexley branch's Equalities officer for members with disabilities. I'm also now the interim joint branch secretary, co-leading the Bexley branch.

25 years on from the introduction of the Disability Discrimination Act, I'm running this short survey to help us get an idea of the needs of disabled members here in Bexley. We'll be pulling the information together to inform us and to discuss with employers – and particularly to feed into the London Borough of Bexley's ongoing work on an Equalities Strategy.

We won't be including any names or identifying information, but will put answers into groups (e.g. schools) and it might be helpful if we could use quotes - but if you don't wish to be quoted you can let us know at the end of the survey.

The data collected in this form will only be used by UNISON for the purposes outlined above. The survey is run using Alchemer (formerly SurveyGizmo), run on servers at their data centre in the European Economic Area or UK. The data will not be sent outside of the EEA/UK. This data will not be shared with third parties beyond the ways outlined above, and it will only be stored for as long as UNISON needs it. It will not be used for any other purposes.
